We’ll be happy to answer your questions and share information about our programs, residences and costs, as well as guide you through the admissions process. Please note that residents must be age 18 or older, must have a psychiatric diagnosis, must admit themselves voluntarily and must be prepared to participate fully in our program.
Typically admission starts with a series of phone calls with the prospective resident, family members and recent providers, which helps our clinical team gather preliminary information to determine if ClearView is the right place for treatment.In addition, we will likely ask for the following information to be emailed or faxed:
- Most recent psychiatric evaluation
- Most recent history and physical
- Most recent psychosocial history
- Most recent clinical notes (if in a treatment program)
- List of current medications
- Other clinical or educational records (IEP plans) that may be applicable
Clinical records can be faxed to 301-378-0113 or emailed to email@example.com. Please be sure to download, complete and return to us the Release of Information Authorization Form, which is required for sharing information.
Effective rehabilitation begins with a thorough assessment to determine the functioning level and goals of each individual. Assessment visits for potential residents are recommended, and family members, friends or other caregivers are encouraged to join in the visit as well. A tour of the residences, office and downtown Frederick will help potential residents and their families get a sense of the overall program setting and community. Program details and fees will also be reviewed at the time of the visit. Following the visit, our clinical team will meet to review the information gathered and make a decision regarding whether or not to offer admission to ClearView Communities.
ClearView is a private payment program. Our fees are kept low thanks to partial funding through the S.C. Herman Foundation.Because many insurance companies do not offer residential treatment benefits, we require payment in advance. However, we will help you talk with your insurance company to assess the possibility of payment for our services, and see if you may be able to recoup some costs of treatment.
24/7 Therapeutic Residential Program ($15,000 per month)
Includes a private room, groceries, weekly dinner out, weekly session with a psychiatrist, weekly session with a therapist, all group therapy and learning activities, and transportation to work, school, medical appointments and shopping. The staff-to-resident ratio is 1 staff member to 4 residents.
Step-Down / Semi-Independent Living ($9,700 / $7,000 per month)
Includes a private room, groceries, weekly dinner out, weekly session with a psychiatrist, weekly session with a therapist, all group therapy and learning activities, and transportation to work, school, medical appointments and shopping. Live-in staff provide up to 40 hours of ongoing support per week.
Supported Living Services ($2,500 per month)
Includes weekly session with a psychiatrist, weekly session with a counselor and participation in group therapy and activities. Please note this is for program participation only. No room or board included.
